Healthy Sacramento Coalition’s Communities of Focus

The following information is from the Community Health Assessment of Sacramento County, prepared by Valley Vision, Inc. for the Healthy Sacramento Coalition, September 2012.

An analysis of both primary and secondary data revealed that there are 15 zip codes in Sacramento County experiencing a high burden of disease.

These 15 zip codes had consistently high rates of poor health outcomes (above county, state and Healthy People 2020 benchmarks) related to chronic disease and mental health.

These 15 zip codes fall into three geographic areas in Sacramento County—North Area, South Area and Downtown.

North Area

Zip codes: 95660, 95673, 95815, 95821, 95838, 95841
Communities (all or part of): Arden Arcade, North Highlands, Foothill Farms, North Sacramento, Rio Linda, Elverta

South Area

Zip codes: 95817, 95820, 95822, 95823, 95824, 95828, 95832
Community (all or part of): South Sacramento

Downtown

Zip codes: 95811, 95814
Community (all or part of): Downtown


North Area map

The North Area is made up of zip codes 95660, 95673, 95815, 95821, 95838 and 95841 and is home to more than 180,000 residents. This area encompasses all or part of the communities of Arden Arcade, North Highlands, Foothill Farms, North Sacramento, Rio Linda and Elverta. Though the communities differ in composition and layout, some of these area zip codes had the highest chronic disease rates in Sacramento County, and were twice and three times higher than county and state benchmarks for many chronic health conditions.


South Area map

The South Area includes seven zip codes – 95817, 95820, 95822, 95823, 95824, 95828 and 95832 – that encompassed all or part of the communities of East City, Land Park, Pocket, Meadowview and South Sacramento. Home to nearly 250,000 residents, it has roughly 20% of the county’s population. Each of these communities consistently exceeded both county and state benchmarks, many by significant amounts, for chronic disease, mental health-related issues, tobacco use and violence/safety markers.


Downtown map

The Downtown area consists of two zip codes, 95811 and 95814, and is home to approximately 25,000 residents. Overall, zip code 95814 has some of the worst rates on health outcome data, socio-demographic indicators and safety data in Sacramento County.
