Health Shack Personal Health Records Project
In 2008, we funded a pilot project called Health Shack, a web-based electronic personal health record and resource site designed to improve health access and outcomes for marginalized, homeless and systems-based youth in Sacramento County. Health Shack is a project of Wind Youth Services in collaboration with UC Davis Adolescent Medicine, CSU Sacramento Public Health Nursing, Linkage to Education and FollowMe, Inc.
Health Shack, which was designed by a collaborative team of youth and adults, provides a safe location for youth and their authorized care providers to store and access important health and other personal information. Once enrolled, members receive a login and password for the Health Shack web site, where they and care providers can enter information into their account and easily access it whenever needed. Public health nurses assist in developing the baseline health records for youth receiving services through this program. Our start-up funds supported development of the initial technology phase, supported youth input into the web design and enrollment of youth in the program. We continue our support as a project partner and funder.
